Completing a Common Assessment Framework (CAF) process

If you are considering carrying out a Common Assessment Framework process, then this may be a useful guide.

Single Service Request Guidance and Single Service Request Form
The new Single Service Request form replaces previous referral forms.
It prompts practitioners to consider the five Every Child Matters outcome areas for children and young people and, if more than one area of need is identified, the practitioner should then complete a Common Assessment Framework. (For CSF, the Single Service Request form has also replaced the old referral form CSF 3155)

CAF form
Changes have been made to the Common Assessment Framework (CAF) form.
The CAF can be used by a practitioner from any agency to assess a child or young person's additional needs. It must be completed alongside the child/ young person and their parent carer (where appropriate).

CAF Action Plan
This should be completed once a Team Around the Child(TAC)has met. It should say who will be doing what and by when. It can also be updated as information changes.

CAF Review Form
This form sits alongside the CAF action plan and should be used to record what progress is being made. It should also be updated when new targets are identified.

Each of these forms should be completed in close liaison with the child/young person and their family.

Reminder: All CAFs (Common Assessment Framework) and TACs (Team Around the Child) MUST be logged with the CAF administrator on 01438 737575.

Forms can be completed by hand or electronically, then printed off.

E-learning
E-learning packages are a great way to brush up on skills and knowledge around integrated practice and information sharing at your own pace. They can be used as a refresher if you have attended integrated practice training or as an introduction to integrated practice if you have not.

Integrated Practice Networks for Practitioners are available in North Herts once a month.

CAF and SSR forms available on SIMS

CSF teams working closely with school staff members who need to access CAF and SSR forms should be aware that both the CAF and SSR forms are available as SIMS reports, which can save school teams time because they eliminate the need to look up and then type in, information regarding contact names and addresses etc.

These reports are available as downloads from the SIMS site on the Grid (HGfl) at
